I hereby consent to the use of the likeness, voice, and/or name of ___________________________, a school student, by the NNN, for all purposes of education, instruction, or public information.  I further consent to the use by NNN, for all purposes of education, instruction, or public information, of any statements made by the student concerning NNN or NNN-funded programs.  I understand that NNN's use of the student's likeness, voice, and/or name, or of the student's statements concerning NNN or NNN-funded programs, may be in publications or broadcasts on any medium, including newspaper, magazine, television, radio, promotional material, on the World Wide Web, or any other medium.  I hereby waive any and all rights that may arise in connection with such use by NNN.

I hereby release NNN, and its Trustees, officers, employees, and agents, from all liability related to NNN's use of the likeness, voice, name, and/or statements of the above-named student for all purposes of education, instruction, or public information, on any or all of the media described above.

También consiento al uso por parte del NNN para cualquier fin educativo, de instrucción, o de información pública, de cualquier declaración que yo pudiera hacer sobre programas del NNN o sobre programas financiados por el NNN. Entiendo que mediante el uso que el NNN pueda hacer de mi retrato, voz y/o nombre o de mis declaraciones referentes a programas del NNN o sobre programas financiados por el NNN, los mismos pueden aparecer en publicaciones o difusiones en cualquier medio, incluyendo periódicos, revistas, televisión, radio, material promocional, en Internet o en cualquier otro medio. Renuncio por este medio a cualquiera y a todos los derechos que puedan presentarse en relación con tal uso por parte del NNN.
